I/We  the undersigned hereby certify that I/We (The Client) am/are the parent(s) or legal guardian(s) of the camper. This agreement is between Snow Soccer Camps Inc (Snow Soccer) and The Client.

Consent for medical treatment:
The Client hereby gives permission for the staff of  Snow Soccer to seek appropriate medical attention for the camper and for the medical attention to be given and for the camper to receive medical attention in the event of accident, injury or illness. The Client will be responsible for any and all costs of medical attention and treatment, except for that covered by the camp’s excess medical coverage policy.

Athletics Release: 
The Client hereby acknowledges that our child is physically fit and mentally capable of participating in soccer camp activities. The Client hereby acknowledges that playing or practicing soccer is a potentially hazardous activity. The Client agrees that The Client is voluntarily entering and assuming all risks associated with participating in those activities, including but not limited to falls, contacts with other participants, the effects of weather including heat and or humidity, and the conditions of the training grounds and facilities, all risks being understood and appreciated by you. The Client, the undersigned for ourselves, our heirs, executors and administrators hereby waive, release, and forever discharge Snow Soccer’s staff, officers, agents, employees, representatives and successors and assign of and from all rights and claims for damages, injury or loss to person or property which may be sustained or occur during participation in Camp activities or while at camp, whether or not damages, injury or loss is due to negligence. The Client also hereby acknowledge that no refunds will be given due to any type of injury occurring during the camp.

Refund Policy
The fees are non-refundable, however the total amount you originally paid for is guaranteed and the value of the registration will be credited towards a later camp or it can be transferred to another player (valid until 12/31/2024).

 

Disputes/Chargeback Policy: By electronically signing this agreement, The Client agrees that due to the no refund policy (credits are sometimes issued) Client agrees not to dispute any authorized charges. However, if The Client does dispute the authorized charge, The Client agrees to pay all collection costs Snow Soccer incurs including court costs, reasonable attorney’s fees, a $75 court appearance fee (each time a Snow Soccer Representative appears in court). The client agrees that if Snow Soccer prevails in court they forfeit the opportunity to use a credit for another Snow Soccer Camp, transfer the credit to another player. The Client also agrees to forfeit any discount received and pay the non-discounted amount for the Snow Soccer Camp. If The Client is found to have fraudulently charge backed or issued a bad check, The Client Agrees to treble damages including in the case of a default judgement. The Client agrees to pay interest at the rate of 21% (or the highest rate allowed by law) from the date of the chargeback to the date of final payment .If Snow Soccer prevails in court, client agrees to pay all post judgement collection costs if judgement amount is not paid within 30 days.

 

Limit of Liability: The Client agrees that the maximum amount of damages The Client is entitled to in any claim of or relating to this agreement or services provided herein are not to exceed The Client’s total amount paid as set forth in this Agreement.

 

General:

 

1) The Client has executed this agreement in its individual capacity and acknowledges that it has read and understands all of the terms and conditions of this document and the client agrees to be bound by its terms.

 

2) If any provision of this Agreement violates the law or is unenforceable, the rest of this agreement will remain valid.

 

3) The Client personally guarantees the payment of the aforesaid purchase price balance in the event of a breach of this agreement and will be liable for all expenses or costs incurred by Snow Soccer in enforcing it’s rights and terms of this agreement.

 

4) By electronically signing this agreement, The Client agrees that due to the no refund policy (credits are sometimes issued) Client agrees not to dispute any authorized charges.

 

5) This agreement shall be governed by the laws of Indiana. The Client consents to the jurisdiction and venue of any court located in the county of Monroe (Indiana) for all matters relating to this agreement.

 

Use material for promotional purposes:
The Client hereby gives permission to Snow Soccer to use pictures and videos of the campers to promote future events organized by the company.

Weather Policy:
In the event of inclement weather once camp begins, all campers will be asked to leave the field to the designated shelter area.  If camp is cancelled once started, you will be asked to return to the field and pick up your player.  We do not have indoor facilities to house players for the rest of the day.
